5. The requirements will show again, just hit the continue button and click PROCEED.

6. You will be redirected to the set your appointment page were you can set your preferred date of appointment. The system will notify you if your chosen date was not available and advise you to choose alternative site.

7. Once done, you need to confirm and print the Philippine passport application form. Check your email to confirm the transaction. Find the email of the DFA and click the confirmation link, you have 24 hours to do so or else your appointment will be cancelled.
